As part of our dedicated team, you'll have the opportunity to spearhead Performance Improvement activities, ensuring the highest standards of care while maintaining fiscal responsibility. Your role will involve not only developing, planning, and implementing improvement initiatives but also meticulously reviewing documentation to guarantee the appropriateness of care and compliance with regulations.

Your expertise will be pivotal in educating and guiding clinicians to provide exceptional care, setting immediate and long-term patient goals, and fostering effective communication through patient care conferences. Additionally, you'll play a crucial role in addressing staffing, client concerns, and ensuring seamless coordination among all stakeholders.

To thrive in this role, you'll need:

- An Associate or Bachelor’s degree in Nursing.
- Current, unencumbered licensure as a Registered Nurse.
- At least one year of supervisory or management experience in a healthcare setting.
- Experience as an RN in Home Health or Hospice with a solid understanding of regulatory compliance.
- Current CPR certification.
