POSITION SUMMARYThe Integrated Health Care Clinician provides clinical and case management services for children and adults across the lifespan in an integrated health care setting that provides primary care, behavioral health and dental services. Demonstrates knowledge of psychopathology and primary modalities of psychotherapy, including individual, group and family. Correctly triages clinical emergencies, including suicide and danger to others. Uses best practices and evidenced based practice models for psychotherapy where appropriate. Makes effective use of topic oriented, focused, time-limited group therapy.Provides or refers to group when required. Demonstrates knowledge of the use of psychotropic medications. Completes clinical documentation in timely manner. Engages in facilitating group (outpatient or intensive outpatient) as well as seeing patients across the lifespan when necessary.Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for identified service needs in a timely manner.Communicates in a positive, effective manner with client, family, and all internal and external providers. Attends any required meetings and multidisciplinary treatment team planning meeting.Meets direct service productivity goals as defined by program. Demonstrates knowledge of program/department/agency goals and target outcomes, and adheres to practice standards and to data collection and reporting protocols.Documents all client related information in compliance with agency policy.Attends and is prepared for scheduled supervision. Seeks additional supervision or consultation as needed and follows through with supervisory directives.Works with program manager or designee to develop, monitor, and analyze clinical and program performance and outcomes and develops and monitors quality improvement program goals.Interfaces and collaborates with community providers to provide positive system development for clients and families, to promote Wheeler Clinic's reputation as a quality service provider.Provides culturally competent care and maintains sensitivity to and respect for clients' cultural traditions, values, and beliefs.Reports child abuse/neglect as mandated, adheres to confidentiality guidelines and ensures that the program meets all necessary licensing, regulatory, and accreditation standards, as related to the program, staff, records, facility, and care of youth.Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
