Willamette Vital Health is Hiring a Caregiver - Tokarski Home - Noc Shift Near Salem, OR

Tokarski Home Caregiver - NOC Shift

When receiving hospice care in one’s own home is not an option, Willamette Vital Health offers the Edward F. Tokarski Home for our hospice patients who need a place to call home in their final months of life.

As a caregiver, you will play an important role in the life of a hospice patient and their family by providing expert personal care to hospice patients in the Tokarski Home (a five-bed adult foster care home). You will bring dignity and compassion to patients in their final weeks and months of life.

NOC Shift (additional shift pay provided)

  • Monday through Thursday - 11:00 pm to 7:30 am (32 hrs per week)
  • Plus occasional on-call

Requirements:

  • High school diploma or GED required.
  • Three years of experience providing direct care to adults who are older or adults with physical disabilities and who require full assistance in four or more activities of daily living.
  • Must be able to lift and carry up to 50 pounds, push up to 200 pounds in a wheelchair, and pull up to 100 pounds.
  • Ability to frequently bend, twist, kneel, sit, stand, climb stairs, and make repetitive motions involving hands/wrists and feet involved in writing and driving.
  • Must have reliable transportation, valid driver's license, and proof of auto insurance.
  • Must be fully vaccinated against COVID-19.

Responsibilities:

  • Provide personal care services to patients under the supervision of the Clinical Manager.
  • Assist in developing individualized care plans as part of the hospice team.
  • Relate to patients and families in an open, empathetic and supportive manner while maintaining personal and professional boundaries.
  • Responsible for electronic charting for patients.
  • Assist with light housekeeping duties as needed.
  • Empower family caregivers by providing information they need to care for their loved one confidently.
  • When necessary, provide personal care services to Tokarski Home residents, assisting in the development of individualized care plans, medication management and charting for each resident, and assist with meal preparation and housekeeping.
  • Identify safety issues and take appropriate actions.
  • Recognize emergencies and institute emergency procedures; or notify appropriate people.

Job Type: Part-time

Pay: From $17.50 per hour

Expected hours: No less than 32 per week
