Job Description
and directs field staff & clinical office staff activities; is responsible for field staff competencies, and their safe and efficient comprehensive home health care delivery to patients. The manager supervises a variety of clinical roles which most often include nurses, aides, social workers, physical therapist, occupational therapist, and dieticians.

CORE PERFORMANCE REQUIREMENTS may include but are not limited to:
- Monitors and maintains productivity standards for all field staff positions.
- Demonstrates effective problem solving, good judgment, and confidential handling of sensitive information.
- Demonstrates accurate and timely documentation in accordance with established regulations.
- Facilitates the establishment and maintenance of successful Clinician/Patient relationships as evidenced
QAPI staff on trends and needed education.
- Communicates professionally and courteously with patients/families, colleagues, and supervisors, along
- Assists management team in facilitating successful licensing, certification and accreditation
- Oversees the scheduling and/or team assistant, assisting with scheduling to ensure appropriate
- Ensures that every patient receives an individualized plan of care, along with coordinating appropriate
- Follows agency Clinical Management model for managing SOC admit assessments, coding requests & Oasis/HIS
- Adheres to applicable State and Federal regulations governing home health, and Agency policies and procedures
- Graduation from an approved school of professional nursing.
- Current licensure by the state of California as a Registered Nurse with at least 2 years of experience in home health/hospice or community health within the last 5 years.
- Demonstrated knowledge of and ability to apply advanced principles of nursing care.
- Demonstrated leadership ability, teaching skills, and the capability to work effective with all levels of personnel.
- Must be C.P.R. Certified.
