Job Summary:
The Patient Navigator works within a team of Navigators, it is a patient advocate and is responsible for guiding patients, families, and caregivers to help overcomes healthcare system barriers and facilitates timely access to quality health and psychosocial health care throughout the continuum of care for breast and lung patients. The Patient Navigator will assist with access issues, develop relationships with referring health care providers and ensure the complete and efficient management of all referrals, including the acquisition of necessary patient-care materials and medical records.
Duties/Responsibilities:
(95%) Patient Care
  • Develop relationships with community groups to create a referral system for support for patients, increase health awareness and provide education
  • Tracks all new patient requests, provide monthly statistics and referring patterns
  • Works closely with the patient care team and assists in the coordination of patient care by removing barriers that would otherwise prevent patients from receiving health care services
  • Support patients by resolving questions, comments and concerns; specifically communicating with each and every biopsy patient both before and after procedures as outlined within Windsong procedures
  • Communicate with newly diagnosed patients within Windsong outlined intervals after diagnosis and support with scheduling procedures, connecting with surgeons, community resources and support services and assist in coordination of services throughout the continuum of care.
  • Obtain images and reports and distribute second opinions to Radiologists
  • Research and stay abreast of new patient resources and treatment options.
  • Work directly with Windsong physicians for directives on Breast Patient Navigator and Lung screening program initiatives and goals.
  • Interact on routine basis with patients, other departments and referring physicians on breast care and lung cancer screening concerns.
  • Promote communication between patient and healthcare providers and reinforce physician-patient relationship.
  • Assist and promote smoking cessation programs for patients.
  • Upload, review and maintain in a tracking system for all patients screened with a low dose screening CT scan.
  • Assist in maintaining the lung registry.
  • Organize and enter breast and lung pathology into reporting system.
  • Enter patient information into lung history forms.
  • Track patients with breast atypia results.
  • Maintain tracker of core-guided breast biopsy patients.
  • If accessible, obtain reports, scripts, and images of patients from outside facilities coming to our facility for core-guided breast biopsies.
  • Contact and assist scheduling patient overdue for annual screening exams (breast and lung).
  • Obtain a script for screening patient, if screening patient does not have health insurance, assist patient with connecting to New York State Cancer Services Program.
(5%) Completes other tasks and projects as assigned
Required Knowledge, Skills, and Abilities:

Education and Experience:
  • High School Diploma or equivalent required.
  • At least two years of work experience in a healthcare environment dealing directly with patients required.
  • Technical clinical experience and/or experience in the area of the practice is a plus
  • Experience and competency in a patient advocacy and/or customer service role
  • Familiarity with medical terminology required
  • Certification from the National Consortium of Breast Centers required within two years
