Wingspan Care Group is Hiring a Medical Provider - PRN Near Shaker Heights, OH

Seeking Medical Provider- PRNJob Summary:The Physician is responsible for providing medical services at our Applewood Centers location on Friday mornings. The Physician will treat all ages and conditions, as appropriate for the setting, and make appropriate referrals as needed. This role is part of a larger interdisciplinary team and will consult with other providers, guardians, and stakeholders as appropriate to provide a high level of patient care.
Essential Duties and Responsibilities:
  • Conduct physical assessments, perform and interpret diagnostic tests, diagnose and treat common conditions, provide screenings, interventions, and counseling, and prescribe medications as needed to patients.
  • Conduct patient medical history and make appropriate assessments, evaluations, and interventions based on the patient’s demographic information.
  • Provide accurate assessment, diagnosis, and treatment of common family illnesses.
  • Educate patients regarding their health and illness prevention.
  • Demonstrate sensitivity to the population served and the individual needs of the patient.
  • Act as a member of interdisciplinary team to develop acute patient care plans, case management plans, and utilization reviews.
  • Observe and monitor patient reactions to medications and procedures.
  • Ensure patient information, visit history, refer outs, lab follow-up, phone calls, and consults are carefully and completely documented in the patient’s record.
  • Ensure adherence to department’s standards and practices.
  • Assist in the development of and/or develop policies and procedures.
  • Plan and regularly re-evaluate patient care needs.
  • Initiate plan of treatment and make necessary revisions.
  • Assist with daily operations of the Wellness Center or the designated health clinic as assigned.
  • Report all suspected cases of child abuse and/or neglect in accordance with Agency policy and state law.
  • Participate in Quality Improvement and Performance Improvement activities and committees as indicated.
  • Comply with all HIPAA and other regulatory and licensing requirements.
  • Keep the exam room and the Wellness Center clean, stocked, and orderly as needed.
  • Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.
  • Provide physical medical management and care in the Wellness Center or at an affiliate location as assigned.
  • Work with the Director of Health and Medical Services to develop, implement, and monitor clinical activities in the Wellness Center and at the Wingspan Care Group affiliates.
  • Attend scheduled staff meetings, supervision, and on-going training.
  • All required trainings, certifications, and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.
  • Respect the privacy of clients and hold in confidence all information obtained during the client’s treatment. All client-related documents should be handled in accordance with Agency guidelines on confidential material.
  • Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.
  • Other duties as assigned by management.
Education/Licensure/Skills/Experience requirements:
  • Graduation from an accredited school of medicine with a M.D. or D.O. degree.
  • Must have an active state medical license in the state of OH.
  • Maintain active BLS certification.
  • Professionalism: Demonstrate a commitment to carrying out professional responsibilities, adherence to ethical principles, and sensitivity to diverse patient populations.
  • Patient Care and Procedural Skills: Provide care that is compassionate, appropriate, and effective treatment for health problems and to promote health.
  • Medical Knowledge: Demonstrate knowledge about established and evolving biomedical, clinical, and cognate sciences and their application in patient care.
  • Practice-Based Learning and Improvement: Able to investigate and evaluate their patient care practices, appraise and assimilate scientific evidence, and improve their practice of medicine.
  • Interpersonal and Communication Skills: Demonstrate skills that result in effective information exchange and teaming with patients, their families, and professional associates (e.g., fostering a therapeutic relationship that is ethically sound, uses effective listening skills with nonverbal and verbal communication; working as both a team member and at times as a leader).
  • Systems-Based Practice: Demonstrate awareness of and responsibility to larger context and systems of healthcare. Be able to call on system resources to provide optimal care.
  • Experience in family medicine or internal medicine with previous experience in pediatrics.
