Winston Prouty Center for Child and Family Development is Hiring a Family Support Worker - Children's Integrated Services Near Brattleboro, VT

The Winston Prouty Center for Child and Family Development is seeking a Family Support Worker to join our Children's Integrated Services (CIS).

CIS Family Support works with families who are expecting a child and families with children up to age 6 through direct services such as home visits, clinical assessments, and assistance connecting with needed community resources.

CIS Family Support works with families who are expecting a child and families with young children through direct services such as home visits, clinical assessments, and assistance connecting with needed community resources. This position will be embedded in the Brattleboro Primary Care practice and will be a pivotal connection to the CIS Clinical team, which includes Early Intervention, Nursing, Early Childhood Family Mental Health and Specialized Child Care.

The Family Support Worker will provide professional support, role modeling and advocacy for families who may be faced with life stressors, such as acquisition of parenting skills and continuing education, and difficulty maintaining basic needs, such as housing and transportation. Promote family success in attainment of life skills with a concentration on promoting optimal health.

Responsibilities:

  • Work with families to define goals and create a plan in collaboration with other providers who may work with the family.
  • Assist families in goal implementation which might include finding housing, employment, early care/ education, transportation, benefits, education, or other resources.
  • Build capacity by identifying family strengths and natural supports
  • Monitor progress towards goals and adapt plan as needed
  • Work collaboratively with other service providers including providing consultative support and providing team leadership when appropriate.
  • Maintain program records.

Qualifications:

  • Bachelor’s degree required in social work, counseling, psychology or related field;
  • Experience working with families with young children as a case manager
  • Ability to establish strong working relationships with program participants, colleagues, and community members.
  • Knowledge of community resources including benefits programs, social services, recovery-oriented support groups, and housing.

Job Type: Full-time

Pay: From $21.00 per hour
