Job Summary The Social Worker/Care Manager position serves as the “go to person” for social services who monitors and evaluates the quality and appropriateness of social services furnished to Witham patients. This position will periodically audit the quality of social services provided by an established process, which will include a “peer review’ process by another Social Worker/Care Manager (licensed); preferably cross unit.
The Social Worker/Care Manager shall serve as a liaison for the physicians, nursing staff and ancillary departments. This person will have a primary duty to provide clinical services for inpatients and outpatients, extended care unit residents; including the provision of individual, group, and family treatment/care conference sessions as indicated. The Social Worker/Care Manager assist people by helping them cope with and solve issues in their everyday lives, such as family & personal problems and dealing with relationships. Social workers help patients and their significant others who face a disability, life-threatening disease, social problem, such as inadequate housing, unemployment or substance abuse. Social worker/Care Manager provides psychosocial support to individuals, families, or vulnerable populations so they can cope with chronic, acute, or terminal illnesses. As part of the discharge planning process they also advise family caregivers, counsel patients, and help plan for patients’ needs after discharge from hospitals. They may arrange for at-home services, such as meals-on-wheels or home care. Social workers may run support groups for patients, families and staff members. This position will interact and fulfill the role of discharge planner. This individual will promote continuity of patient care with physicians and long term facilities to ensure that patient needs are addressed in a timely manner. This individual supports the achievement of optimal patient outcomes and unit operations by promoting individual growth, team functioning, and interdepartmental cooperation within the framework of shared decision-making and coordinated care. This individual must demonstrate the Witham Health Services philosophy of caring in all aspects of job performance and interpersonal relationships.
Minimum Education/License/Certification Requirements

• Licensed Clinical Social Worker (LCSW) preferred, who has completed a Master’s Degree in Social Work from an accredited school of social work or Bachelor’s degree in Social Work or Sociology with the intent and commitment to obtain Master’s in Social Work (preferably LCSW) from an accredited school within a specified amount of time directed by Human Resources.
• Minimum 3 years previous clinical experience in a hospital setting, working collaboratively with physicians, SNF setting or behavioral health setting experience may be required also depending on position need at time. .
• Must be CPR certified for all positions and CPI certified if working on Behavioral Unit.
