Description

Job Summary:

The Supported Living Case Manager is responsible for the coordination of service that promotes independent living and maximizes self direction through a system of service supports for individuals with disabilities in their own apartment and/or community living.

Qualification/Education Experience:

  • BA and/or 3 – 5 years related human service experience.
  • Possession of a valid Massachusetts Driver’s License, safe driving record, as per Work Inc policy and use own vehicle to transport individuals.

Essential Functions

  • Provide Case Management services to individuals per Supported Living Service Plan.
  • Consistent recordkeeping as required as assistance to individuals on benefit entitlements, check writing, etc
  • Assists individuals in community integration to the fullest extent possible.
  • Assists individuals on any PCA issues that arise where the consumer may require some counseling, advice, etc. May also act as surrogate
  • Trains the consumer in all aspects of supervision etc in order to manage PCA’s independently.
  • Provides emergency back-up services during non-work hours as required.
  • Preparation, implementation and participation in the DDS ISP process and MRC SLSP process.
  • Assists individuals in advocating for their rights and rights of other disabled persons.
  • Oversees any additional tasks as delegated by Program Director.
  •  Attends all trainings as required
  •  Follows all applicable Federal/State regulations and Agency Personnel Policies

Qualifications


Supervisory Responsibilities:

  • Directly supports up to ten individuals.
  • Ensuring the health and safety of each individual.
  • Carries out supervisory responsibilities in accordance with the Work Inc.’s company policy and applicable laws.
  • Responsibilities include interviewing, hiring, training, and record keeping for PCA’s
  • Planning, organizing, training and directing work.
  • Addresses complaints and resolving problems

Mathematical Skills:

  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent, ability to create and interpret graphs.

Reasoning Ability:

  • Ability to define problems, collect data, establish facts and draw valid conclusions.
  • Ability to interpret an extensive variety of technical data in mathematical or diagram form and deal with several abstract and concrete variables.

Communication Demands:

  • The ability to read, analyze and interpret journals, financial reports and legal documents.
  • The ability to respond to common inquiries of complaints.
  • The ability to effectively present information to the Board of Directors, employees, clients and members of the business community.

The work is performed in a non-smoking environment. The work is normally performed in a temperature controlled zone, however, there are exceptions.   • Good standing balance, defined as the ability to stand and walk without a loss of balance.
  • Full upper extremity range of motion defined as the ability to obtain:
  • Full shoulder flexion, extension, abduction, and rotation.
  • Full elbow flexion, extension, supination and pronation.
  • Full wrist flexion, extension, radial and ulnar deviation.
  • Normal upper extremity strength defined as the ability to move the specified body part through a full range of motion against maximum resistance.
  • Full lower extremity range of motion defined as the ability to obtain:
  •  Full hip flexion, extension, abduction, adduction and rotation.
  • Full knee flexion and extension.
  • Full ankle flexion and extension.
  • Normal lower extremity strength defined as the ability to move the specified body part through a full range of motion against maximum resistance.
  • Full trunk range of motion as defined by the ability to obtain full flexion and extension, rotation and side bending.
  • Full cervical range of motion as defined by the ability to obtain full flexion and extension, lateral flexion and rotation.
