Role: The clinician plays a key role in oversight and treatment of the wound care patient.
Responsibilities include but are not limited to:
**LVNs can perform all tasks delegated to them within reason by the RN, Physician and/or PA-C.**
Develops and implements wound prevention, skin management, and wound treatment programs and provides corresponding education to patients, family members/caregivers, and facility/agency staff.
· In conjunction with prescribing providers orders (MD/DO, APRN, PA), provides comprehensive patient and wound assessments and reassessments, treatment plan recommendations, bedside patient wound care and patient education.
· May provide the following wound care without an order: Dressing and cast removal, cleansing, irrigating, probing, ABI, neuropathy assessment
· Provides bedside wound care to include application of primary dressing, secondary dressing, compression bandages and wraps, wound vac placement, offloading devices, removal and preparation of fiberglass cast, aid provider in placing fiberglass cast. This may include chemical cauterization with a provider order, per facility guidelines and if allowed according to individual state practice act.
· Must have validated clinical competency prior to independent application.
As an interdisciplinary wound care team member, collaborates to establish and implement individualized, comprehensive care plans that promote wound healing and prevention.
· Observes patient’s response and wound status, reporting any changes to the provider or supervising clinician, according to facility or agency guidelines.
· Provides and reinforces education to patients, family members/caregivers, facility/agency staff regarding preventative measures, interventions, and individualized patient treatment plans.
· Communicate with home health agencies and provide orders, education and clinical updates on mutual patients.
· Order labs, imaging, referrals, wound care supplies and medical records as requested/prescribed by the provider. Ensure order receipt, scheduling and obtain results for provider review.
· Provide patient updates on pertinent changes to their wound care plan, test results and answer all patient questions and concerns.
· Attend and actively participate in daily staff huddles in a constructive manner.
