Overview:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will have a passion for helping families in need. This position offers the opportunity to make a positive impact on the community by providing support and guidance to individuals facing various challenges. Helping families find funding and enrolling in the Adult Day Center.
Responsibilities:
- Conduct assessments to determine clients' needs and develop individualized care plans
- Provide information for admission and offer referral services
- Assist participants/clients or caregivers in obtaining financial assistance when needed
- Prepare and provide caregiver support meetings, arranging speakers, assigning topics for discussion and providing information for assistance
- Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive care
- Advocate for clients' rights and access to services
- Proof admission paperwork scan to appropriate staff for review
- Coordinate referrals to community resources and services
Skills:
- Conflict management
- Experience working with Adults with disabilities
- Knowledge of family law and adult protective services
- Crisis management skills
- Background in social work or related field
