We seek a skilled and compassionate Licensed Practical Nurse (LPN) to join our dynamic and caring patient-centered team in our outpatient offices! - Both Adult and Pediatric Positions Available

Job Overview:

This is a full-time on-site role for a Licensed Practical Nurse at Wyckoff Heights Medical Center. The LPN will be responsible for providing direct patient care, administering medications, assisting registered nurses and physicians, monitoring patient vital signs, updating medical records, and ensuring our patients' overall comfort and well-being. The LPN will work closely with the healthcare team to deliver quality care to patients.

Duties will include:

Assisting the physician in treatments and procedures

Monitoring of patient's vital signs, patient intake and output, and recording patient history

Reports data relating to patient's health status to appropriate clinical staff

Identifies patient goals, communicates, and collaborates with clinical team to support patient care plan

Administration of medication, injections and functions in the scope of the license

Cleaning of equipment in patient care areas

Maintain patient care rooms to ensure they are kept clean, and supplies are well stocked.

Maintain records of patient activities and observations

Submit records to the appropriate clinical staff regarding patient's condition

Assist with scheduling patients, authorizations and referrals to support office staff as needed

Cross-cover multiple practices

Rooming patients

Overseeing patient flow

Skills Required:

Knowledge of the insurance process

High level of professionalism

Flexible and team-oriented - Attention to detail and quality

Ambulatory practice experience - PREFERRED

Qualifications Minimum Requirements:

• Graduate of an accredited school of practical nursing

• Current NY State LPN License and Registration

• BLS Certification
