Responsibilities
- Deliver culturally competent social work services to patients and their families to enhance their well-being.
- Advocate for patients to ensure they have access to necessary healthcare services and assist in overcoming obstacles.
- Attend community events as necessary to promote our services and support the local community.
- Maintain professionalism, integrity, and autonomy in your work.
- Utilize knowledge of community resources and entitlement programs in care planning.
- Maintain accurate patient care records according to company policies.
- Represent the department in hospital committees and workgroups.
- Collaborate in the enrollment process by gathering complete client referral information.
- Provide billing information in both Chinese and Korean.
- Assist clients in accessing government assistance programs such as Medicare, Medicaid, housing support, and childcare services.
- Perform any other duties as required.
- Job Type: Full-time
- Salary: $40,000 - 45,000 per year
- Benefits: Health insurance, Paid time off
- Schedule: 8-hour shifts, Monday to Friday
- Expected hours: 40 per week
- Experience in Microsoft Excel and Homecare preferred
- Proficiency in Korean and Chinese required
- Social Worker Certificate preferred
- Location: In-person at Bayside, NY (relocation required)
