The Intake Specialist primary responsibilities include conducting thorough screenings, ensuring accurate data collection, and referring youth and families to the appropriate services. This role reports to the Teen Center Director and requires strong communication skills, a compassionate approach, and the ability to work collaboratively with others. \
Requirements
- Create a welcoming and inclusive environment for youth entering the Curfew Center, establishing rapport, and building positive relationships.
- Conduct comprehensive intake assessments for all youth entering the Curfew Center and develop individualized plans to support the transition of youth from the curfew center to the regular teen center program.
- Gather and record accurate demographic information, including personal details, contact information, and emergency contacts.
- Ensure privacy and confidentiality of all clients and collected information.
- Collaborate with internal teams and external service providers to identify appropriate resources and referrals based on the assessed needs of the youth.
- Provide guidance and support in connecting individuals to necessary services, such as counseling, educational programs, in-house services, community resources, or legal assistance.
- Provide ongoing support and guidance to youth, encouraging their active participation in teen center program and activities.
- Maintain up to date knowledge of available inhouse services, community resources and maintain a referral directory to facilitate efficient service navigation.
- Maintain accurate and complete records of all intake assessments, screenings, service referrals and youth engagement plans.
- Enter data into electronic databases or management systems in a timely and accurate manner.
- Prepare and submit regular reports on intake and referral statistics as required by Xiente and/or DHS and the City of Philadelphia.
- Work collaboratively with the teen center team, including case managers, counselors, Teen Center Director, and other program staff, to ensure coordinated and comprehensive services and to facilitate a smooth integration of youth into existing activities and programs
- Attend team meetings, case conferences, and training sessions to enhance knowledge and contribute to the overall effectiveness of the program.
- Other duties as assigned.
Qualifications and Experience Requirements:
- Bachelors Degree and 2 years of experience as intake, social work, case management, counselor or other closely relevant experience
- Must have high proficiency with computer software, including Microsoft Word, Excel and
- Outlook. Advanced skills in Microsoft Excel is a must.
- Current PA state Criminal Clearance, DHS Child Abuse History Clearance, FBI Fingerprint, Health Assessment with Mantoux TB Shot, National Sex Offenders Registration Clearance

- Possess or ability to secure PA Driver's License and insurance (preferred).
