Job Title: Enrollment Coordinator (Chinese/Korean Speaking)

Xtreme homecare is looking for caring and talented people to join our Social Worker Team. We know that putting people in need of healthcare services together with a dedicated homecare company with excellent and motivated caregivers to improve quality of life is right up your alley.

Responsibilities:

• Provide a variety of social work modalities to patients and their families with culturally competent direct service that helps individuals maximize their functioning.
• Advocates on the individual's behalf to achieve maximum access to health care, identifies hurdles, and proposes solutions.
• Participate in community events as necessary.
• Function independently, with personal integrity and ability to work effectively.
• Incorporates understanding of entitlement schemes for community resources and sand into every planning.
• Adheres to site policies and procedures for maintaining accurate professional patient care records.
• Represents the department and profession in various hospital/site-based committees and workgroups.
• Obtain and submit complete potential client referral information for enrollment process.
• Provide bilingual billing information.
• Maintains a positive and professional demeanor toward clients, visitors, families, and co-workers.
• Performs other duties as assigned.
• Assist clients in accessing government assistance programs, such as Medicare, Medicaid, Section 8 housing, and childcare services
• Medicaid Processing, Food Stamp, Senior Housing, SSI, or etc...

Bi-Lingual - English and Chinese/Korean

Job Type: Full-time

Salary: $40,000 - 45,000 per year

Schedule:

• 8 hour shift
• Monday to Friday

Expected hours: 40 per week

Experience:

• Microsoft Excel: 1 year (Preferred)
• Homecare: 1 year (Preferred)

Language:

• Korean (Required)
• Chinese (Required)

License/Certification:

• Social Worker Certificate (Preferred)

Ability to Commute:

• Bayside, NY (Required)

Ability to Relocate:

• Bayside, NY: Relocate before starting work (Required)
