Zepf Center is Hiring a Case Manager-MOMS Grant Near Toledo, OH

Description

Zepf Center has been serving the Lucas County community for nearly 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff.

We are currently seeking a full-time Case Manager for our MOMS Grant to provide ongoing wraparound integrated care coordination services to an assigned caseload of adult consumers, primarily who are enrolled in the MOMS Program. This position is to provide a wide range of community support and coordination of services to pregnant women and people experiencing drug and alcohol and mental health concerns and to ensure their needs are addressed. The Case Manager Specialist assists clients with the acquisition of community resources and the development of short and long-term goals. Services will be provided with respect and in accordance with client’s rights.

Essential Duties and Responsibilities:

  • Ability to work with minimal supervision and the ability to adapt quickly to patients changing needs.
  • Provide therapeutic client-centered intervention included but limited to, service coordination group/family counseling, recovery planning, as well as participation in care coordination/team meetings.
  • Assesses client strengths and needs. With the client, family and applicable team members, develops a comprehensive care coordination and treatment plan. Monitors and evaluates client progress toward goal attainment. Reviews and modifies plans every 90 days.
  • Maintains contact with the client as indicated in the treatment plan or as needed to insure stability at least every 90 days or less if required by regulatory bodies. Communicates with and provides support to clients in achieving goals and outcomes. 
  • Links and/or refers clients to other services, programs, and agencies as needed. Assists in insuring clients obtain and retain applicable entitlements and follows up on same. Serve as an advocate for clients. With appropriate releases, communicates with family members and significant others as indicated.
  • Confers with supervisor and other team members regarding clients and as appropriate, with emphasis on provision of most appropriate, effective and cost efficient services. 
  • May provide face-to-face contact with clients who are hospitalized to facilitate discharge planning and coordination of services. Work with hospital and agency medical staff regarding same. 
  • Assists clients in tracking and keeping appointments. 
  •  Provides back up for other staff.
  •  Ability to write clear, accurate and concise notes/documentation in accordance with all confidentiality laws and standards as well as agency requirements. 
  • Carries out policies and procedures of the agency; may participate in community work, staff development, agency-wide projects; serve on agency committees/teams, attend meetings and performs other related duties as requested. 
  • Perform other duties as assigned. 

Requirements

  • CDCA required.
  • Knowledge of the MOMS decision tree and related trainings required by the grant. 
  • Those eligible to drive company vehicles must have a valid driver’s license and be eligible for coverage as defined by the agency commercial insurance carrier. Those who drive personal vehicles in the course of business must be able to provide proof of insurance as necessary.
