Join Our Team as a Mental Health Case Manager
OverviewAre you passionate about providing community-based mental health services to patients in Vancouver, WA? Our top client is seeking a dedicated Mental Health Case Manager to join their team. In this role, you will work closely with patients to create and achieve individualized service plans that address their unique needs. Reporting to the Mental Health Services Program Director and Program Supervisor, you will collaborate with a treatment team to deliver comprehensive case management services.
Key ResponsibilitiesCreating and implementing individual service plans to meet patients' unique needs
Collaborating with a treatment team to provide holistic care
Delivering community-based case management services in alignment with service plans and evaluation recommendations
Coordinating referrals to and from other programs for seamless patient care
Maintaining accurate clinical files to track services and interactions
Networking within the community to connect patients with relevant resources
Establishing and nurturing relationships with community agencies and service providers
RequirementsBachelor's degree in mental health counseling or a related social service science (in progress accepted)
Washington State Department of Health credentials
Minimum one year of experience working with individuals facing co-occurring mental health and substance use disorders or in a mental health treatment facility
Familiarity with Washington State regulations governing mental health services
Additional InformationFull-time position with a possible hybrid schedule
Generous benefits package, including 4 weeks of accrued paid vacation and tuition reimbursement
Competitive hourly pay ranging from $18.75 to $19.75
