Exciting Opportunity in Vancouver, WA for Mental Health Case Manager Our client in Vancouver, WA, is actively seeking a Mental Health Case Manager to provide community-based mental health services to patients in need. This individual will work under the guidance of the Mental Health Services Program Director and Program Supervisor to create and implement individual service plans tailored to each patient's unique needs. Joining a dedicated treatment team, the Case Manager will offer community-based case management services, facilitate referrals to and from other programs, and maintain comprehensive clinical documentation. Key Responsibilities
Collaborate with patients to develop and achieve individualized service plans
Contribute to a multidisciplinary treatment team
Provide community-based case management services, aligning with service plans and evaluation recommendations
Facilitate referrals between programs for optimal patient care coordination
Maintain accurate and up-to-date clinical records
Establish a network with community resources to support patients' needs
Cultivate relationships with external agencies and service providers to enhance patient care
Requirements
Pursuing or holding a bachelor's degree in mental health counseling or relevant social service science field
Washington State Department of Health credentials
Minimum of one year of experience in working with individuals facing co-occurring mental health and substance use challenges, or in a mental health treatment setting
Understanding of Washington State regulations governing mental health service provision
