Overview:
We Are Seeking a highly Organized Program Administrator to join our team. As a Program Administrator, you will be responsible for overseeing and managing various programs within our organization. This is a vital role that requires strong leadership skills, excellent communication abilities, and a passion for making a positive impact in the community. In conjunction with the Office Administrator; will supervise, coordinate, and manage all of the Waiver Services, (and all HCBS) populations administration of paperwork, and quarterly meeting with the Support Coordinators. Coordinate Children's Choices Services and implementation.
Duties:
- Responsible for Day to Day Operations of Satellite/Branch Office
- Hire and Train Direct Service Workers Staff
- Conduct Intake Process On New or Potential Clients
- Complete Quarterly reports and Provider Plan of Care for all NOW Waiver consumers
- Supervise Satellite/Branch Office Direct Service Workers Field Staff
- Perform community outreach activities to assist community based physicians, Home Health Agencies, and/or medical clinic staff to educate eligible members of disease management services that would be eligible for Long Term Care options.
- Contacting providers of care as needed, to ensure proper continuum of care to current consumers.
- Leading in the "hosting" of an information table at common community area event locations.
- Serving as a resource to staff members.
- Maintain consumer files according to waiver record keeping regulations
- Educating community members about available community resources and assisting members in utilizing the resources.
- Complete all other duties and activities assigned by the Administrator and Quality Assurance Specialist
Experience:
Minimum Job Qualifications:
Must have a valid driver's License and liability insurance coverage.
Must be able to Pass Criminal Background Check
Must Meet One of the following Qualifications:
- a) Bachelor’s degree, plus a minimum of four years of verifiable experience working in a field providing services to the elderly and/or persons with developmental disabilities; or
- b) A minimum of six years of verifiable experience working in a health or social service related business, plus a minimum of four additional years of verifiable experience working in a field providing services to the elderly and/or persons with developmental disabilities; or
- c) An RN licensed and in good standing with the LSBN and have at least two years’ experience in providing care to the elderly or to adults with disabilities.
