Interview patients or their representatives to identify problems relating to care; Communicate with care home supervisor regarding all new clients and changes in staff assignments; Explain policies, procedures, or services to residents and their families using medical or administrative knowledge; Monitor high risk residents to provide extra assistance with health education, care coordination and follow-up; Investigate and direct patient inquiries or complaints to appropriate staff and follow up to ensure satisfactory resolution; Coordinate communication between patients, family members, medical staff, administrative staff, or regulatory agencies; Interview all prospective support employees; Ensure employee files are complete with all required documentation in place; and Provide general information to the public regarding the care home's services.
