Targeted Case Manager
Qualifications for Position:
Must have a bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field.
To work with the Children's population, A case manager must have a bachelor's degree from an accredited university or college and three years full time or equivalent experience working with children with serious emotional disturbances.
Has completed or agrees to complete AHCA-approved mental health targeted case management training within three months of initially providing Medicaid services. If the training is not completed within three months, the provider agency cannot continue to bill Medicaid for services rendered by the case manager under the supervisor's Medicaid provider number.
Have knowledge of available resources in the service area for children with serious emotional disturbances.
Be knowledgeable of and comply with state and federal statutes, rules and policies that affect the target population.
Job Duties:
Provide Person-centered assessment and service plan development for Medicaid clients who are eligible for Case Management services in the adult and children targeted population
Linking and coordination of services based on client's individualized needs and available resources, including but not limited to psychiatric monitoring, medication management, linking with transportation services, meals, clothing, and legal services as needed.
Monitoring client's progress and reassessment and follow-up as needed
Focusing on resiliency and encouraging independence in clients
Working with the client and the client's family to address issues related to implementation of the service plan. Services where the family is involved must clearly be directed to meeting the identified needs of the client.
Assessing the effectiveness of the service plan in meeting the identified needs of the client.
Advocating for the acquisition of services and resources necessary to implement the service plan by representing or defending clients through direct intervention. 8. Coordinating the delivery of services as specified in the service plan with the help of the client, the client's family, and the client's natural support system. Monitoring service delivery to evaluate the client's progress.
Documenting mental health targeted case management activities in accordance with the documentation requirements in the Case Management Provider Handbook.
Crisis Intervention/Support by assisting clients in crisis in getting access to the necessary resources in order to cope with the situation.
Arranging for and coordinating after care services upon discharge from a residential or inpatient facility when discharge planning is not covered by the facility's per diem staff.
When needed and appropriate, may participate in the client's individualized treatment plan development or individualized services plan review under the Medicaid community behavioral health services program.
