Responsibilities:
The Patient Care Coordinator provides clinical support to A Better Homecare patients primarily through phone encounters. They answer inbound inquiries from patients, triaging and communicating to providers and staff with the appropriate urgency. They communicate clinical information to patients and staff for coordination of care. The Patient Care Coordinator is a member of the patient’s care team, assisting with necessary care coordination efforts and proactively monitoring patients upon instruction.
Essential Duties and Responsibilities
- Provide medical triage, consistent with scope of practice and competency, for inbound clinical inquiries (via telephone calls) , including inquiries related to new / emerging medical conditions or sick calls, requests for new orders, and questions about medications
- Maintain a paneled caseload with a mixture of high and low acuity patients and will be responsible for tasks such as care plans, case conferences and monitoring.
- Utilizes appropriate triage protocols when assessing a clinical situation and providing recommendations to patients and providers
- Document all phone calls and patient interactions in the EHR promptly by utilizing standard clinical documentation guidelines as required by A Better Homecare and local, state and federal rules and regulations
- Review, transcribe, communicate and processes providers' verbal orders, as requested
- Assist with patients' coordination of care, via telephone requests and fax requests, including acting as a liaison for clinical field staff.
Qualifications:
Required Knowledge, Skills and Experience
- Ability to react decisively and quickly in emergencies
- Knowledge of medications effects, indications, and contraindications
- Experience with multi-tasking
- Proficiency in using electronic health records (EHR) system, and Microsoft Office Suite
- Ability to communicate effectively with patients and their family members, and facility staff
- Knowledge of basic principles, practices and techniques in home healthcare
- Ability to read and interpret written information, and write clearly and informatively
- Ability to speak clearly and persuasively in positive or negative situations, including listening and getting clarifications, and responding well to questions
Preferred Knowledge, Skills and Experience
- Active RN/LPN/LVN license in the state where patients are located
- 2 years of experience as an RN/LPN or LVN
- Familiarity with Medicare and Medicaid insurances and programs
- Familiarity with home health and hospice
- Experience in Kinnser/Wellsky (EHR)
