We are seeking a dedicated Client Services Coordinator to support clients and ensure the smooth operation of intake and assessment processes. This role is critical to our mission of delivering exceptional care and outcomes.
Responsibilities
- Oversee and execute the client intake process, ensuring accurate patient verification, assessments, teaching, and timely scheduling of clinical/medical services.
- Provide program related transportation service for pickup and drop off.
- Retrieve informed consent and release of information documents.
- Conduct and complete biopsychosocial and medical assessments for new clients.
- Manage intake checklists and input client information into our database, including ICD-10 codes, medical history, medication profiles, and care plans.
- Collaborate professionally with medical staff, therapists, hospital liaisons, case managers, and other stakeholders.
- Participate in daily team meetings and contribute to effective client care.
- Use attention to detail and knowledge of regulatory compliance to ensure accurate documentation and follow-ups.
- Set up client-facing software programs and technology, including patient portals, breathalyzers, iPads, and Fitbits.

Job Type: Contract
Pay: $22.00 - $25.00 per hour
