SUMMARY
The case management process requires Case Managers to have specialized skills and competencies to perform, at minimum, five core functions. The core functions form an on-going and dynamic case management process. Case Management core functions are: 1) Comprehensive Assessment; 2) Service Planning; 3) Service Plan Implementation; 4) Monitoring; 5) Reassessment
QUALIFICATIONS
The minimum qualifications for Case Managers are: 1. RN with one year paid professional experience 2. LPN with one year paid professional experience 3. Baccalaureate degree and one year paid professional experience with aging or disabled population, performing duties which encompass the core functions of case management.
DUTIES AND RESPONSIBILITIES
As a service coordinator, the Case Manager facilitates interdisciplinary team process to:
- Identify consumer needs and strengths;
- Identify and coordinate service delivery;
- Arrange for and initiate service implementation;
- Monitor service delivery;
- Re-evaluate need and goals and change services plans as necessary;
- Properly document in consumers’ file; and
- Prepare written reports as necessary and required.
As an advocate, the Case Manager ensures that:CM
- Consumers receive appropriate quality service;
- Services are modified to meet changing consumer needs;
- Interventions are made to ensure the system is responsive to consumers; and
- The plan of care is progressively moving the individual toward self-care and preventing further loss of function.
As a consumer consultant, the Case Manager:
- Assists consumers to recognize and identify their service needs;
- Educated consumers on the quality and appropriateness of services;
- Supports consumers to take responsibility for their own care to the greatest extent possible.
- When necessary, represents consumers in interactions with DHS program administration.
- Reports any suspected abide, neglect, or exploitation of any consumer to the Agency and any other appropriate agencies.
As a gatekeeper, the Case Manager assures that:
- Only those consumers appropriate for the services through the DHS/Advantage Programs receive the appropriate services.
- To the extent possible, ways are built into the service plan to progressively move the consumer toward independence and the use of informal, family, and volunteer service;
- Expenditure of funds is justified; and
- Costs are monitored and contained.
As an agency staff member, the Case Manager will:
- Coordinate on an on-going basis with Case Management Supervisor and/or Backup Case Management Supervisor, and Administrator.
- Maintain at all times confidentiality of clients, consumers, patients, employees, and agency business and abides by HIPAA regulations;
- Review and enforce Agency’s Policy and Procedure Manual;
- Assist in orientation of new staff;
- Attend staff meetings and in-services;
- Assist the continuous Quality Improvement Administrator with regular review and audits of consumer records to insure quality, accuracy, and completeness;
- Other duties and responsibilities as assigned by Case Management Supervisor and/or Backup Case Management Supervisor, Administrator, and/or the Advantage program.
- Will provide assistance, if requested, to the Administrator in the extenuating circumstances, including but not limited to, Holidays and Surveys
