Case Manager III for Developmental Services

Job Title: Case Manager III / Support Coordinator III (ID Case Management)Job Summary:

The Support Coordinator helps individuals with intellectual disabilities (ID) access essential services like medical care, education, housing, and job support. They assist in developing personalized support plans and ensure individuals receive the services they need to live independently in the community.

Reports To: Program AdministratorSupervision: NoneWork Conditions:

• Requires home and agency visits
• Flexible schedule
• Some travel outside Hampton Roads may be needed

Job Duties:

• Assess individuals’ needs and create support plans.
• Connect individuals with services like Medicaid Waiver programs and community resources.
• Maintain accurate documentation for compliance and billing.
• Meet with individuals at least every 90 days to track progress and adjust plans.
• Monitor service delivery and individual satisfaction monthly.
• Help individuals access emergency services (e.g., housing, food, medical care).
• Assist other units when needed for training or administrative tasks (not direct clinical services).

Requirements:

• Education: Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (Master’s preferred).
• Experience: At least 2 years of experience in case management, social services, or working with individuals with disabilities.
• Certifications: Must meet state certification requirements for case management (if applicable).
• Skills:
• Strong communication and organizational skills
• Ability to develop and implement support plans
• Knowledge of Medicaid Waiver programs and community resources
• Proficiency in documentation and compliance with state regulations
• Other Requirements:
• Valid driver’s license and reliable transportation
• Ability to pass background checks and required screenings
• Ability to work flexible hours, including occasional travel

Job Types: Full-time, Contract

Pay: $24.00 per hour

Expected hours: 40 per week
