Case Management Team Lead
We are seeking a candidate with supervisory experience to join our team. The Case Management Team Lead will be responsible for supervising a team of case managers and to ensure the proper delivery of case management services to our patients. The candidate will Monday-Friday from 9:00am-6:00pm(1 hour lunch) CST in a professional, fast-paced environment.
**Responsibilities:**
· Understand and implement best practices in Texas Medicaid case management.
· Supervise a team of 10-15 case managers.
· Scheduling initial assessments
· Assist case manager with developing and implementing treatment plans and treatment plan reviews.
· Assist case manager with developing strategies to engage clients.
· Assist case managers with obtaining CANS and ANSA.
· Review notes, treatment plans, timesheets, discharges and patient surveys.
· Link clients and case managers to community resources.
· Provide weekly and monthly individual and group supervision.
· Enforce company policies and procedures.
· Complete 90 day and annual staff evaluations
· Assist team with eliminating Fraud, Waste, and Abuse
· Attend conferences, daily quality control, and team meetings.
· Develop, attend, and present mandatory training.
· Attend and report outcomes at daily meetings.
· Perform other duties as assigned.
**Skills:**
- Strong leadership skills in managing patient care
- Experience in managed care and utilization review processes
- Ability to create comprehensive care plans tailored to individual patient needs
- Proficiency in discharge planning procedures
- Knowledge of Medicid regulations and reimbursement processes
- Familiarity with medical records management
- Ability to multitask

Job Qualifications:
· A bachelor’s degree from an accredited college or university with a minimum number of hours that are equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
Job Type: Full-time
Pay: $20.00 - $25.00 per hour
Expected hours: 40 per week
