Description
The Intake Clinic Coordinator is responsible for the day-to-day coordination of intake services to home health and hospice patients. This position works to coordinate the care of patients, new patient referrals and resolve patient problems as they arise.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Serves as intake coordinator for home health and hospice referrals and screens for appropriateness for location, program, diagnosis, SOC date and patient needs.
- Requests patient records, orders as needed, RC codes, and verifies PMD prior to service.
- Coordinates with clinical assistant for insurance verification, a service agreement, and eligibility.
- Enters patient information, medications, and other necessary information into patient chart.
- Completes eligibility paperwork and sends to medical director for verbal orders, if needed.
- Coordinates assignments of new referrals to clinicians.
- Ensures patients come on to service, are turned down, or orders are in order for a later admission.
- Ensures all medications, equipment and medical supplies are ordered in a timely and cost-effective manner prior to admission.
- Coordinates preadmission and admission plans.
- Provides field staff with office support by taking lab reports, taking MD orders, following-up on MD faxes with case manager, etc.
- Troubleshoots patient care problems as they arise.
- Thoroughly documents all clinical coordination activities.
- Works closely with physicians and other health care providers, offering recommendations related to the provision of health care services
- Assists in the development, implementation, and revision of clinical standards and procedures as needed.
- Maintains knowledge of current trends in home health and hospice.
- Participates in staff meetings, QI meetings, management meetings and other meetings as assigned.
- Maintains up-to-date knowledge of State and Federal home health and hospice regulations.
- Provides other support duties as needed

Minimum Qualifications
- Minimum of one to two years of clinical home care and/or hospice experience required.
- WI CMA, LPN, or RN
Knowledge, Skills and Abilities

- Working knowledge of Microsoft Office (Excel, Word, PowerPoint, Outlook).
