Our relationship-based care model is designed to provide coordinated health care through a curated network that improves the quality and outcomes in the communities we serve. Our member-centered approach focuses on individualized care to meet the unique needs of our members and is supported by our dedicated care team of primary care physicians, health coaches, nurses and an array of specialized services. We strive for clinical excellence and aim to increase quality, increase patient satisfaction and reduce unnecessary utilization of health care resources. AdventHealth Senior Care is creating a new connection around how whole-person care is delivered to better meet the full spectrum of health care needs in this senior population. AdventHealth Senior Care is looking for a Care Coordinator to be responsible for delivering outstanding experiences and high-touch assistance for our members. The Care Coordinator builds individual relationships with our members to ensure they are happy with their experience. The primary responsibilities of the Care Coordinator center around greeting members, scheduling appointments, managing member referrals, updating medical records and answering inbound/outbound phone calls. The Care Coordinator should make a lasting impression and play an important role in AdventHealth Senior Care's team-based care approach.

The Value You’ll Bring To The Team
- Welcomes members appropriately, accurately updates, verifies and enters member demographics (including insurance and billing) and processes/scans all forms into appropriate systems.
- Provides education as necessary to member regarding test locations, portal use, and follow-up (non-clinical) actions.
- Documents appropriately within the office’s designated system to ensure regulatory requirements are met.
- Schedule and confirm member appointments.
- Answers the telephone and handles calls in a timely and friendly manner including appropriate documentation of the call.
- Obtains appropriate medical records and member information to support patient care needs and documents within the EMR accurately and timely.
Minimum Qualifications
- High School graduate or equivalent
- Graduate of an Accredited Medical Assistant Program
- Certified Medical Assistant (CMA)-AAMA or
- Registered Medical Assistant (RMA)-American Medical Technologists or
- American Medical Certification Association (“AMCA”) for Clinical Medical Assistant Certification (CMAC) or
- National Association for Health Professionals (“NAHP”) for Nationally Registered Certified Medical Assistant (NRCMA) or
- National Center for Competency Testing (“NCCT”) for National Certified Medical Assistant (NCMA) or
- National Healthcareer Association (NHA) for Certified Clinical Medical Assistant (CCMA)
