Advanced Practice Providers -Lake County, CA

AHCL and its Live Well Institute (LWI) is seeking a mission minded advanced practice provider (APP) who is looking to make a difference in the population served. Experience desired is someone who endorses lifestyle medicine principles and teaches the importance of adopting these principles to best manage chronic disease. Having chronic disease management (CDM) experience (3 years) is of high importance as is the ability to communicate effectively with Medi-Cal and Medicare patients. In addition to its wellness offerings, The LWI looks to elevate its DM service line… specifically for diabetes, heart disease, COPD & asthma. Patterned after evidence-based programming elsewhere (e.g. Joslin Diabetes Center), the LWI desires to not only improve the health status of Lake County residents, but looks to publish its successes in peer reviewed journals.

Adventist Health Clear Lake (AHCL) is a 25-bed Critical Access medical center in Clearlake, California, serving Lake County. Services include 24-hour emergency care, diagnostic imaging (MRI, CT mammography), home care, ICU, laboratory, maternity, physical rehabilitation, pulmonary rehabilitation, surgery, and ten rural health clinics around the lake that provide primary care, behavioral health, dental care, OB/GYN, pediatrics and specialty health care services.
In 2019, AHCL had 1,352 admissions, 159 deliveries, 22,364 emergency visits and 66,960 outpatient visits. Our Rural Health Clinics had 149,021 visits. The medical center is staffed by more than 40 physicians, 571 employees and 51 volunteers.

Wage Scale: $125,000-$170,000/year
