Job Summary
The Assistant Program Coordinator will be responsible for assisting in the planning, implementation, and evaluation of various programs within the organization. This role requires excellent communication skills, the ability to multitask, and a passion for making a positive impact in the community. The Assistant Program Coordinator provides oversight and supervision of independent apartments serving individuals with physical and mental health disabilities. The Assistant Program Coordinator is responsible for programs, services, staff, and physical sites and ensures continuous compliance to all federal and state regulations. The Assistant Program Coordinator provides leadership to the staff teams at each site and serves as a liaison between support team members and community agencies while ensuring compliance with all county and state regulations. This position also provides direct care to individuals as needed. All duties are subject to policy, procedure and management directives, and may change at any time.
Duties
Complete resident reports, ensuring compliance with licensing requirements and agency standards.
- Implement resident programs focused on safety, independence, and community integration.
- Supervise and coordinate site cleaning to maintain a home-like appearance.
- Perform monthly safety checks, report repair needs, and manage snow/ice removal and cleaning tasks.
- Provide phone consultations to staff as needed.
- Assist and train staff in delivering services according to resident plans and outcomes, including observing their work to assess competency.
- Audit case files regularly to ensure accurate service documentation.
- Maintain record-keeping systems and implement correction plans when necessary.
- Work with support teams and consultants to resolve issues with contracted services.
- Schedule and coordinate appointments for residents' health and consultant services.
- Manage and collect rental payments from residents.
- Oversee staff scheduling and ensure all authorized service outcomes are delivered, including behavior management and daily living activities.
- Monitor program site maintenance, utilities, and neighborhood relations.
- Document all service delivery with measurable data.
- Promote community integration and recreational opportunities for residents.
- Keep management informed of resident progress, concerns, and incidents.
- Maintain confidentiality and report safety or maltreatment concerns immediately.
- Ensure Direct Support Professionals follow their job descriptions and promote resident safety.
- Cover open shifts as needed
- Lift up to 50 lbs when required
Qualifications must meet one of the following:
1. Associate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
2. A diploma in a field related to human services from an accredited post secondary institution and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
3. A minimum of 50 hours of education and training related to human services and disabilities;
4. Three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
Job Type: Full-time
Pay: $21.00 - $22.00 per hour
