What are the responsibilities and job description for the Caregiver Support Coordinator position at Aging Office of Western Nebraska?
Overview
This program determines the needs of Caregiver’s and Clients linking them with appropriate in-home services that will enable them to remain independent for as long as possible. This position includes Caregiver Technology & Equipment, Caregiver Information & Assistance . Coordinator must meet annual goals and objectives, data collection, analysis, and reporting for all the programs.
Skills
· Must report to work as scheduled on a regular and reliable basis.
· Must have good computer skills with general knowledge of Microsoft office and can learn other computer systems.
· Develop and maintain systematic process for the referrals for all programs. Process referral requests in a timely manner.
· Assistance to caregivers in gaining access to services.
· Individual counseling, organizing of support groups, and caregiver training to assist caregivers.
Respite care to help temporarily relieve caregivers from their caregiving responsibilities.
· Set up and Maintain Emergency Response Systems, inventory, records management, and client.
· Ensure client's knowledge and understanding of program benefits, rights, and responsibilities.
Qualifications
High School graduate and two years’ experience in Human Services working with the elderly.
Proven experience in data entry and computer application.
Proficient telephone and communication etiquette skills
Job Type: Full-time
Pay: $18.00 - $20.00 per hour
Expected hours: 40 per week
