High Plains Crossing, Greeley's premier Memory Care community, is looking for a dedicated Health Services Director to join our leadership team. You will be responsible for managing the day-to-day healthcare operations of the community to ensure residents’ healthcare needs are met. Additionally, the Health Services Director supervises the licensed nurses and other direct care staff in the community to ensure residents receive quality care while being treated with respect and dignity.

Duties and Responsibilities:
- Responsible for the direct supervision of community based licensed nursing staff (LPN/LVN, RN), the QMAPs, and the Lead Resident Care associate
- Assesses health, functional and psycho-social status of residents, initiates individualized service plans, proactively manages care and services for each resident, and evaluates the effectiveness and maximizes the resident’s opportunity to remain their environment
- Manages the health care of residents including communication with families and associates
- Assigns and directs the work of team members
- Provides training, supervision, and monitoring of team members in the administration of medication and situation specific training
- Participates in pre-admission screening of prospective new residents, as well as on-site evaluations of residents admitted to alternate care environments for treatment
- Routinely evaluates residents and documents changes in their condition, then notifies team members, physician, and resident’s family of any changes
- Communicates with physicians regarding residents’ status and condition changes. Ensures residents are prepared for physical appointments
- Ensures in-house ancillary medical services are scheduled and completed
Qualifications:
- LPN/LVN or RN license
- Minimum 3 years of relevant experience
- Previous management experience
- Ability to exercise independent judgment and discretion
- Basic computer skills, preferably in a Microsoft Windows environment
- Due to the nature of the work performed by Ascent Living Communities, and that this position works with a vulnerable population, we do require a criminal background check be completed
Pay:
$80,000 to $85,000 annual salary, based on experience
