What are the responsibilities and job description for the Patient Navigator CCH position at Alivio Medical Center?
Summary
The role of the navigator is comprehensive onboarding and education for each prenatal patient to orient them of the CCH services for prenatal care, women’s health and pediatric care. The overall goal is to retain and support education for their adherence to care for favorable outcomes; well woman, prenatal, postpartum and pediatric. The navigation team educates the patient on the patient centered care team approach to support and resources. The navigation team links and follows up with patients on resources. Additionally, they serve as the clinic support to their delivery plan to Stroger Hospital.
Typical Services
- Cordially greets patients and guests, ascertains their needs, and directs them
accordingly.
- Provides timely escort services to incoming patients and visitors to designated
destinations throughout CCH.
- Onboarding and education for each prenatal patient to orient them of the CCH services for prenatal care, women’s health and pediatric care.
- Conduct follow up outreach to retain and support education for medical adherence to
care for favorable outcomes; well woman, prenatal, postpartum and pediatric.
- Conducts hospital Labor and Delivery tours and function as a resource for patients and
guests seeking information and assistance.
- Assists patients and guests by responding to inquiries and providing accurate
information about navigating CCH services.
- Conducts data entry and provide follow-up on referrals to linkages to care as needed.
- Perform other related duties assigned.
Patient Navigator Resource Requirements
- High School diploma or equivalent
- One (1) year of healthcare customers service experience
Knowledge, Skills, Abilities and Other Characteristics
- Excellent customer service skills
- Willingness and ability to uphold CCH Rules of Conduct
- Ability to establish and maintain effective working relationships with patients,
employees, and the public.
- Ability to communicate with people from various educational levels and backgrounds
and to speak clearly and concisely.
- Ability to stand and walk for prolonged periods of time
- Ability to travel to other Cook County Health locations based on need.
- Ability to follow detailed instructions.
- Ability to maintain confidential patient information in compliance with the Health
Insurance Portability and Accountability Act (HIPAA).
- Must have the ability to communicate Bilingual (English and Spanish)
Reporting Relationships
The Patient Navigator will directly report to the Patient Navigators Coordinator.
These are fulltime roles; Clinic locations are Mondays – Fridays with rotating Saturdays. Schedule subject to change if productivity demands change. Location can change depending on volume and service demands.
