Position Summary:
We are seeking passionate and qualified health coaches to join our team on a contracted, remote basis. In this role, you will work directly with Alleviant’s psychiatric providers to develop and implement personalized health coaching plans that support patients in achieving their wellness goals. This is an opportunity to apply your skills in an outpatient clinical setting, empowering patients with holistic lifestyle interventions and strategies to complement their medical care.
Key Responsibilities:
• Conduct virtual health coaching assessments using Alleviant’s electronic medical records platform.
• Collaborate with psychiatric providers during patient sessions to assess needs and create individualized coaching plans.
• Access and review patient medical records to prepare for appointments.
• Lead an in-depth health coaching assessment during initial sessions, culminating in a personalized lifestyle management plan.
• Provide ongoing support and education to patients, as directed by the medical provider.
• Document coaching sessions and maintain accurate records within the EMR system.
• Focus solely on health coaching tasks without additional administrative or operational responsibilities.
Qualifications:
• Certification or formal training in health coaching, preferably from an accredited program (Weimar University graduates preferred).
• Strong understanding of holistic health principles, lifestyle interventions, and integrative care.
• Excellent communication and interpersonal skills, with the ability to connect with diverse patients.
• Experience working in healthcare or mental health settings is a plus.
• Comfort with technology, including electronic medical records and virtual communication platforms.
• Ability to work independently while collaborating effectively within a multidisciplinary team.
