You will focus on:
- Chronic and Acute Condition Capture: Proactively prompt providers to ensure accurate capture of chronic and acute conditions for patients with upcoming scheduled for visits (at least 2 weeks in advance).
- Quality Gap Identification: Identify quality gaps in the EMR system for patients, ensuring that missing or incomplete documentation is addressed prior to the scheduled visit.
- Gap Closure Education: Educate providers on necessary gap closures related to patient care and clinical documentation, ensuring they understand the importance of accurate and comprehensive documentation.
- Review of Concurrent Encounters: Review sampled concurrent encounter documentation to verify that care gaps are being addressed and closed effectively.
- Collaboration with Providers and Staff: Build strong rapport with providers and clinical staff to foster a collaborative environment aimed at improving patient care and documentation accuracy.
- Documentation Accuracy: Ensure that all gaps are closed with accurate clinical documentation and proper administrative follow-up, including correct claim submissions for billing purposes.
- Stakeholder Engagement: Maintain continuous communication with key stakeholders involved in patient care, including providers, nursing staff, billing teams, and quality assurance personnel, to ensure seamless gap closure.
- Rotating Office Visits: Rotate through provider offices as assigned, offering on-site support and assisting with the implementation of gap closure strategies.
- Clinical Documentation Interpretation: Use clinical knowledge (LVN background preferred) to interpret and guide appropriate documentation, ensuring that chronic and acute conditions are captured and reported in line with best practices.
- Bachelor’s degree with a minimum license as a Licensed Vocational Nurse (LVN) required.
- RN preferred.
- 3 years of clinical experience, preferably in a medical office or healthcare setting.
- Experience with clinical documentation, EMR systems, and understanding of quality measures, value-based care, or risk adjustment coding.
- Prior experience in quality gap closure or working with providers to improve clinical documentation.
- ICD-10 CM Risk Adjustment experience.
- Understanding of provider office workflows
- 2-3 years working with excel data, word and power point platforms
- Strong clinical knowledge and ability to interpret medical documentation.
- Excellent communication and interpersonal skills, with the ability to build rapport with providers and clinical staff.
- Detail-oriented with strong organizational and problem-solving abilities.
- Ability to work independently and manage multiple tasks efficiently.
- Proficient in using EMR systems and Microsoft Office Suite.
- Ability to educate and guide clinical staff on documentation and process improvement.

The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements. Physical Requirements: Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork – Activity level: Sedentary, frequency most of workday.
