We are seeking a talented and organized Intake Coordinator to assist with our Home Health Care patient referrals and follow up.
Intake Coordinator Job Description:
Our Intake Coordinator provides clerical support for the Director of Clinical Services and clinical staff; demonstrates proficiency in the use of computers and Medicare guidelines; organizes workload and sets priorities to meet deadlines in a fast-paced office; assists with answering a multi-line phone system and communicates with various patients, staff and community resources
Intake Coordinator Responsibilities:
- Responsible for all incoming referrals; including: checking insurance for eligibility, checking location of the patient, entering the patient information into the web-based software system.
- Creates a friendly/caring/cooperative impression with visitors/patients/physicians and those who inquire about AHH services.
- Call physicians/patients to verify information during the intake process.
- Assign nursing and therapies to new patients; notify staff of new patients.
- Answers telephone and channels calls appropriately; takes messages accurately.
- Responsible for submitting OASIS reporting on a weekly basis; fixes any errors if needed; keeps a tracking system of reports.
- Promotes AHH referrals in the community through public relations.
- Creates and maintains a medical record log of all new patients/referrals; sends log to liaison weekly
- Orders and maintains adequate medical supplies for use by field staff; assists clinical staff with medical and non-medical supply acquisition.
- Takes verbal orders via phone and creates a physician order.
- Uploads new orders into patient medical record.
- Sends DME orders to appropriate DME companies.
- Creates and distributes the weekend schedule to the nurse on-call.
- Protects confidentiality of all agency and patient- related information, written or verbal.
- Maintains an acceptable attendance record, reports to work punctually and demonstrates appropriate appearance/neatness/cleanliness; complies with AHH dress code.
- Maintains personal growth/education and demonstrates professional behavior by attending appropriate departmental meetings/in-services.
- Keeps personal area clean/neat at all times; assists with office cleaning/maintenance as needed.
- Follows agency quality assurance, safety and infection control policies and procedures.
- Assists clinical field staff with various needs and requests.
- Additional duties as assigned by the Director of Clinical Services or Executive Director.
Intake Coordinator Qualifications:
- Minimum eighteen (18) years of age
- High school graduate required; additional college or business courses preferred
- Minimum of one (1) year experience in a secretarial position; able to perform all basic clerical functions, i.e. typing, filing, rules of spelling, grammar and punctuation
- Proficient in MS Word and MS Excel data entry
- Familiarity with medical terminology preferred
- Ability to work in a fast-paced office, set priorities and meet multiple deadlines
- Good interpersonal and communication skills
- Satisfactory verification of clearance with the Employment Disqualification List (EDL) and Family Care Safety Registry (FCSR) and/or criminal background check
Intake Coordinator Work Environment:
Works indoors in the home health office and in patient’s homes as needed
