POSITION SUMMARY – Director of Case Management
POSITION DUTIES – Provides overall direction for the clinical care coordination program which includes the coordination, negotiation, and procurement of care of patients to achieve quality and cost effective patient outcomes throughout the continuum of care of the patient which includes all health care settings from acute inpatient treatment through follow up in the office, chronic disease management and preventative care
MINIMUM REQUIRED QUALIFICATIONS –
- OH/compact RN license
- BSN
- AHA BLS & AHA ACLS
- 5 years of experience in case management, discharge planning or other cost/quality management program.
- 3 years experience in a hospital clinical role.
PREFERRED QUALIFICATIONS –
- Previous supervisory/management experience.
- Master's Degree
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – M-F 8a - 5p, Weekends & On Call as needed (40hrs/wk)
