POSITION SUMMARY – RN Case Manager - Home Health

POSITION DUTIES – Screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.

MINIMUM REQUIRED QUALIFICATIONS – Graduate of accredited school of nursing; NH & VT or compact RN license; BLS; 2 years home health case management experience.

LENGTH OF ASSIGNMENT – 16 weeks

SHIFT / HOURS PER WEEK – Day shift - 8:00am - 4:30pm, Monday - Friday w/rotating weekends (32-40hrs/wk).
