What You'll Do :
- Responsible for supervising and guiding clinical staff in the provision of hospice services.
- Screens referrals and assigns patients to appropriate team members.
- Monitors individual caseloads and adjusts daily workload of team.
- Responsible for ensuring that patient care is coordinated, delivered and managed appropriately.
- Reviews staff documentation for completeness, accuracy, timeliness, and quality.
