Position Overview:
We are seeking a compassionate and experienced Clinical Manager to join our dynamic Hospice team. As the Clinical Manager, you will play a crucial role in leading and overseeing the day-to-day operations of our hospice home care services. You will be responsible for ensuring high-quality patient care, supporting a team of healthcare professionals, and promoting a collaborative, compassionate environment for patients and their families during their most difficult times.
Key Responsibilities:
- Supervise and manage clinical staff, ensuring optimal care delivery.
- Monitor patient care and ensure compliance with hospice regulations, company policies, and industry best practices.
- Coordinate with the medical director, physicians, and other interdisciplinary team members to develop individualized care plans.
- Conduct regular clinical assessments and audits to maintain the highest standards of patient care.
- Provide education and training to clinical staff to enhance their skills and knowledge in hospice care.
- Assist with staffing and scheduling to ensure appropriate coverage and continuity of care.
- Act as a resource for clinical staff and families, addressing concerns or questions regarding patient care.
- Oversee and maintain documentation of patient care, ensuring accurate and timely recordkeeping in accordance with regulatory guidelines.
- Participate in the development of policies and procedures to improve hospice care practices and patient outcomes.
- Promote the hospice philosophy of care, ensuring a focus on dignity, comfort, and respect for each patient.
Qualifications:
- Registered Nurse (RN) with current licensure in Maine.
- At least 3-5 years of clinical experience in hospice, palliative care, or a related field.
- Previous leadership or management experience in a clinical setting preferred.
- Strong knowledge of hospice regulations, Medicare guidelines, and clinical best practices.
- Excellent communication and interpersonal skills, with a compassionate and patient-centered approach.
- Ability to work independently, as well as part of a multidisciplinary team.
- Strong organizational skills and the ability to handle multiple priorities in a fast-paced environment.
- BLS/CPR certification required; additional certifications (CHPN, etc.) preferred.
