Job Summary
We are seeking a dedicated and experienced Field Supervisor to provider Compassionate Care to our clients. PHC/FC Program Supervisor duties both on-site and in the field, as necessary, in working with the Primary Home Care (PHC) and Family Care (FC) Program employees and clients. The primary goal is to assist in ensuring compliance and providing impeccable services to the PHC/FC clients.
Position requires your own transportation.
Responsibilities
- Coordinates client care as needed
- Perform Initial/Annual and Supervisory Visits
- Assists with completion of Human resource documents
- Comply with State Regulations
- Data entry of all client visit in a timely manner
- Assist clients in the home as needed according to ISP
- Address any client concerns or feedback promptly and professionally to Director/Administrator
- Maintain client records according to policy and procedure
- Participate in On-Call rotating schedules
- Ability to work well under pressure and meet deadlines
- Performs “Self Reporting” of complaints/incidents involving Abuse, Neglect and/or Exploitation
- Complies with standards of the PHC/FC Programs, the Policies and Procedures and the regulations and requirements of all governmental agencies.
- Performs and helps to maintain EVV Visit Verifications in VESTA for all the program contracts.
Job Type: Full-time
Pay: $15.00 - $17.00 per hour
