Job Summary:The Case Manager at ACCS will be responsible for overseeing and managing client cases with a focus on utilization management, staffing, and quality assurance. This role involves maintaining accurate billing and utilization records, ensuring all caseloads are properly staffed, conducting interviews, and ensuring that all services meet quality standards. The Case Manager will report to the Clinical Coordinator, and work collaboratively with the Admin Team and staff to ensure timely, efficient, and high-quality service delivery.
Key Responsibilities:
- Monitor all client cases to ensure total utilization is within the prescribed limits, avoiding overages and ensuring billed amounts align with services delivered.
- Review and complete weekly utilization reports for accuracy, ensuring all data is correct and up-to-date.
- Identify and report any issues related to caseload authorizations to the Admin Team; escalate discrepancies or concerns to the Waiver Director.
- Collaborate with the Admin Team and Waiver Director to discuss critical revisions, such as the addition or subtraction of service units.
- Assist with conducting interviews for open cases on assigned caseloads, including second interviews and follow-ups from job offer to orientation.
- Create and distribute caseload schedules for new staff orientees to the Admin Team at least one week prior to orientation.
- Coordinate meet-and-greet and shadowing schedules for both new and current staff members to ensure seamless onboarding and integration.
- Provide weekly updates to families, team members, and Support Coordinators (SCs) regarding the progress of staffing unassigned cases.
- Attend Referral Meetings with the Clinical Director to discuss and assign new cases.
- Manage the Open Shifts Tracker, ensuring daily communication about open shift needs through email and GroupMe.
- Oversee the Open Positions Tracker, ensuring daily updates and communication regarding staffing needs are sent to the Admin Team and staff.
- Complete Monthly Program Notes (MPNs) and Quarterly Program Notes (QPNs) for unassigned cases or CS-only cases by the 5th of each month.
- Ensure all staff profiles in Setworks are up-to-date, including consent forms and ISP confirmations, prior to visits.
- Oversee incident management by promptly reporting incidents to the Compliance/Incident Manager (CI), confirming completion of the EIM (Incident Management), and facilitating communication with families.
- Attend Team Meetings and Individual Support Plans (ISPs) as needed to ensure high-quality service delivery and client outcomes.
- Perform weekly Health and Safety Checks for individuals without staff and document findings in Setworks under the client profile ("Additional Info" section).
- Review Setworks session notes for quality assurance, identifying any cases of concern or issues with service delivery.
- Lead and oversee consumer-led events, ensuring high levels of engagement and active participation from clients.
- Assist with the Tech Mentorship Program, providing guidance and support to staff in enhancing their technical skills.
- Send birthday cards to clients as part of fostering positive client relationships and showing appreciation.
Skills & Qualifications:
- Education: A Bachelor's degree in Behavioral Science, Social Science, or a closely related field; OR a Bachelor's degree in any field and one year of human services-related experience.
- Ability and willingness to work in the field as well as in a virtual setting.
- Strong interpersonal and teamwork skills with the ability to work independently.
- Excellent relationship-building abilities, capable of working with diverse individuals and teams.
- Strong organizational, time management, and detail-oriented skills.
- Ability to prioritize urgent matters while maintaining a full caseload.
- Excellent writing, speaking, facilitation, coaching, and computer skills.
- Understanding of Waiver Services and the Office of Developmental Programs.
- Internal candidates must have a track record of timely and accurate documentation, including encounter notes and required HR documentation.
- Strong commitment to maintaining confidentiality and adhering to organizational policies and procedures.
- Ability to handle multiple tasks simultaneously and manage competing priorities.
