Position Summary
The Medication Safety Program Coord works in collaboration with pharmacy leaders and other health care professionals to develop and implement medication safety best practices and activities to ensure safe use of medications across various practice sites and service lines. Evaluates medication adverse events and errors and applies continuous process improvement to ensure patient safety. Assists pharmacy leaders to meet regulatory and accreditation standards. Reports to the Pharmacy Service Line Director
Key Responsibilities
- Serve as a medication safety specialist and expert on best practices for medication safety
- Manage medication error reporting through the review and investigation of all reported or observed events.
- Coordinates pharmacy involvement in collaborative clinical or operational efforts and performance improvement ensuring safe medication use and patient safety.
Position Qualifications
Experience
- 3 years experience in institutional pharmacy practice. Experience in performance improvement practices and/or teams, required.
Education
- Bachelor's degree in Pharmacy, required.
Certification/Licensure
- RPh: Registered Pharmacist by Oregon Bard of Pharmacy, required.
- Licensed Pharmacy Preceptor by Oregon Board of Pharmacy, preferred.
- CPPS: Certified Professional in Patient Safety, preferred.
