The Director of Clinical Services will be responsible for, the implementation, delivery, coordination and supervision of hospice services provided consistent with Ascends' policies and procedures. These duties can include but are not limited to:
- Implements and monitors compliance with agency policies and procedures related to clinical services
- Serves as a member of the clinical management team in order to address problems associated with the delivery of clinical services; identify staff development needs; put forth suggestions for appropriate changes in policies and procedures; participate in Performance Improvement activities; gives peer input to enhance and improve the middle management team.
- Participates in and provides team leadership during regular IDT team meetings.
- Facilitates problem solving sessions and delivers team support.
- Supervises the maintenance of medical records assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.
- Collects and reviews pertinent clinical data at team meetings.
- Revises care plans, completes care plans and charts of discharged patients.
- Monitors documentation of all team nurses.
- Participates in staff selection and orientation of new personnel.
- Conducts supervisory visits to allow for observation of clinical performance of assigned team members.
- Submits written performance evaluations in accordance with policy requirements.
- Provides information for inclusion in written performance evaluations for all staff assigned in team in cooperation with appropriate members of the clinical management team.
- Provides leadership and counsel to clinical personnel to enhance staff development.
- Identifies need for staff education and assists in the development of in-service programs for the IDT team.
- Assists agency in education/training programs.
- Communicates all pertinent clinical and administrative information to the Executive Director and all team members.

Education:
Bachelor's Degree in Nursing
Professional Experience:
- Must have a State Certified RN license
- Certified Hospice & Palliative Care Nurse (CHPN) certification, a plus
- Experience in hospice or home health, preferred
- Minimum of 5 years nursing experience, with 2 years in a supervisory role
- Demonstrated skills in administrative performance, including expertise in written/oral communication, supervision of staff, documentation requirements, service coordination and case management.
