The Referral Coordinator will be essential in managing and coordinating referrals for young clients seeking mental health services. This role requires effective communication and collaboration with CMO’s, clients, families, and mental health professionals, to ensure seamless access to care. The ideal candidate will have a passion for youth mental health, strong organizational skills, and a commitment to making a positive impact in the lives of young individuals. Process and track client referrals to appropriate mental health services, ensuring timely and accurate communication.
Client Engagement: Interact with youth and their families to understand their needs, provide information about available services, and assist them in navigating the referral process.
Collaboration: Work closely with mental health professionals, schools, and community organizations to facilitate smooth transitions and comprehensive support for clients.
Documentation: Maintain accurate and confidential records of referrals, client interactions, and communications in compliance with agency policies.
Resource Development: Stay informed about community mental health resources and services to provide clients with a wide range of support options.
Follow-Up: Conduct follow-ups with clients and providers to ensure successful connections to services and gather feedback on the referral process.
Administrative Support: Assist with general administrative tasks as needed to support the agency’s operations. ---
Qualifications: -
- Bachelor’s degree in social work, psychology, public health, or a related field preferred.
- Previous experience in referral coordination, case management, or a similar role in a mental health or youth services setting is highly desirable.
- Strong interpersonal and communication skills, with the ability to build rapport with youth and collaborate effectively with team members and external providers.
- Excellent organizational skills and attention to detail, with the ability to manage multiple tasks and prioritize effectively.
- Proficiency in Microsoft Office Suite and experience with NJ Performcare systems is a plus.
- Knowledge of mental health issues affecting youth and available community resources is beneficial.
