QIDP at Elite Healthcare
A Qualified Intellectual Disability Professional or QIDP for short. Is a full-time position that is responsible for the day-to-day delivery of individual support program services to assigned caseloads in accordance with Federal, State, and facility codes, regulations, guidelines and expectations. At Elite Healthcare we are family! We love getting the participants together for community activities like bowling, going to the movies, holiday gatherings, etc. We strive to incorporate fun within the Admin team as much as we can!
QUALIFICATIONS:
- Bachelor’s Degree in human services professional, including but not limited to sociology, special education, rehabilitation counseling, psychology, and social work.
- 1 year of experience with an individual with an intellectual or developmental disability.
JOB RESPONSIBILITIES AND DUTIES:
- Attend all client meetings including but not limited to monthly TSC meetings, Liberty Healthcare, annual assessments, or Nurse Assessor Assessments.
- Maintain and update client books as needed/Annually
- Obtain all necessary info for client records including but not limited to service plans, history and physicals, medication administration information, psych assessments, etc.
- Complete client functional assessments 90 days after the client moves in as well as annual
- Complete all new participant intake paperwork, including but not limited to PIP, Implementation Plans, Safety Plans, and gathering all other paperwork from other agencies. As well as writing their new plans.
- Update all clients PIP’s, Implementation Plan and Safety Plans annually
- All client specific training with staff
- Weekly orientation for new hires when scheduled
- All new client PIP's and IP's
- Visiting with clients two times a month. One time could be via phone call, text the other time in person.
- Collecting and QA’ing Data sheets monthly in a timely manner and sending it to the appropriate people when time.
- Taking crisis calls for all participants as needed.
- Turn in all CIR to state within 24 hours
- Building and supporting the Service coordinator (TSC) for our participants. Communicating with them on any changes within the participant’s life.
- Building, maintaining and supporting Guardian as well as involving them in the training of new staff if needed.
- Perform quarterly reviews with participants as well as the living environments
- weekly meetings with administration
- assist administrator with other necessary items
- Assist QA as needed for MAR’s, DSL and Incident Reports

Pay: $21.00 - $25.00 per hour
