Overview
Assured Home Health is seeking a highly skilled and experienced Home Health Clinical Operations Manager to oversee the patient care operations of our home care facility. As a key member of our management team, you will play a critical role in ensuring the delivery of high-quality patient care services that meet the needs of our patients.
Key Responsibilities
- Leadership and Oversight
- Supervise and direct the activities of our clinical staff to ensure the delivery of safe, economical, and efficient patient care.
- Ensure compliance with federal and state regulations, as well as Joint Commission standards.
- Patient Care Coordination
- Coordinate with physicians, families, and other clinicians involved in patient care to ensure effective communication and collaboration.
- Develop, implement, and update individualized plans of care to meet the unique needs of each patient.
- Staff Performance Management
- Supervise and monitor the performance of our agency staff, utilizing tools such as the clinician scorecard and performance evaluation tool.
- Ensure that only qualified personnel are hired and maintain licensure verification, certification, and credentialing as required by law and policy.
- Operational Metrics and Reporting
- Drive operational metrics, including productivity, VBP, SVP, ACH, claims alert, late EMS, unverified services, orders audit, and agency financial statements to ensure alignment with company goals.
- Follow up on issues, address concerns, and implement action plans as necessary to meet goals.
Requirements
- Licensure and Certification
- Current RN licensure in the state of practice (or applicable license for PT/OT/SLP)
- Current CPR certification
- Experience
- One year of supervisory and/or management experience in a home health setting or related healthcare setting preferred
- Education
- Bachelor's degree in Nursing (or related field) preferred
