The Role
We are seeking an experienced Social Services Professional to join our Lakeshore Team. The successful candidate will have a Master's degree in Social Work or a related field, MSW certification, CPR certification, a valid Michigan driver's license, and reliable transportation for home visits.
Responsibilities:
- Conduct in-depth evaluations of clients' health status and living situations.
- Develop and execute personalized care plans that consider clients' unique needs and preferences.
- Provide guidance and support to clients, families, and caregivers on managing daily tasks and planning for the future.
- Act as a bridge between clients and local agencies, facilitating access to necessary resources.
- Offer brief therapy sessions to help clients cope with social and emotional challenges.
- Collaborate with colleagues to ensure continuous care and identify areas where additional support may be needed.
- Play a key role in discharge planning and addressing clients' psychosocial needs.
- Keep detailed records of client interactions.
- Nurture relationships with public and private organizations to leverage available resources.
- Attend training sessions, case reviews, and meetings to guarantee aligned care strategies.
