What are the responsibilities and job description for the Social Services Case Manager 1 (Full Time) position at Aurora San Diego?
To provide direct and indirect patient care and case management services to active-duty patients receiving inpatient care.
*Pay Range: $22/hr. to $25/hr.
Work Schedule: Monday to Friday from 8:00 a.m. to 4:30 p.m.
Essential functions:

- Manage aspects of multiple cases from admit to discharge
- Communication with referral sources as requested
- Work within a multidisciplinary clinical team
- Write patient check-ins and documentation
- Take care of patient needs during their treatment
- Discharge planning assistances
- Attend clinical treatment teams as needed
- Administrative: faxing, filing, report preparation, legal log, etc.
- Bachelor’s degree in Psychology or Social Work or related clinical field
Preferred Education and Experience:
- A minimum of one year work experience pre or post-graduation
- Interest in the field of Behavioral Health
- Strong oral and written communication skills
- Ability to work effectively both independently and as part of an interdisciplinary treatment team
- Knowledge of the difficulties of dealing with chronic mental illness
- Knowledge of community resources appropriate for patients
