1. Pull and forward discharged charts stored in the warehouse as requests come in.
2. Purge warehouse records which are at or past legal retention requirements in preparation for destruction.
3. Close records as needed if the appropriate program has not already closed the records.
4. Maintain the process for retrieval of discharged records in the warehouse.
5. Re-file charts returned to the warehouse in the appropriate storage box.
6. Scan all loose papers, including correspondence, into the medical record of the appropriate program in a timely and accurate manner.
7. Verify that requests for information from outside agencies, facilities, or persons are appropriately completed by the requestor.
8. Respond to appropriately completed requests for information from outside agencies, facilities, and persons complying with Federal and State regulations.
9. Request copies of medical records from outside agencies.
10. Answer phone calls resolving callers problem or forwarding the call to the appropriate person.
11. Process incoming mail.
1. Update knowledge of State and Federal requirements for release of information.
2. Assist with pulling of records past their retention requirements.

2. Attends variety of meetings, conferences, seminars as required or directed.
3. Demonstrates use of Quality Improvement in daily operations.
4. Complies with all health and safety regulations and requirements.

1. 2 years of exposure in a Health Information Management Department.
2. Ability to operate computer, scanning equipment, printers, FAX machines, and scanners.
3. Communicates well both written and verbally with co-workers and clients, ability to function well in a busy department.
